Required Licenses/Certifications:
APRN with Neonatal Nurse Practitioner (NNP) certification or Physician Assistant‑Certified (PA‑C); Basic Life Support (BLS), Pediatric Advanced Life Support (PALS), and Neonatal Resuscitation Program (NRP) within 90 days of hire
Physical Requirements:
lifting, pushing, or pulling objects up to 50 lbs; sitting for long periods of time
Shift:
Monday‑through‑Friday, equivalent to two 12‑hour shifts in the NICU and two 8‑hour shifts in the clinic setting
Job Summary The Advanced Practice Provider (APP) functions within the pediatric chronic care team in an ambulatory setting and within the neonatal ICU to manage the needs of chronically ill patients. The APP provides in‑patient care in the NICU, assists with discharge planning, coordinates home medical devices, and offers post‑discharge follow‑up in the ambulatory setting, including telehealth. The APP works closely with outpatient testing groups, case management, and the inpatient care team to ensure continuity of care for families transitioning to home. The APP is an integral member of a multidisciplinary health care team with a strong foundation in evidence‑based practice, collaboration, and education.
Key Responsibilities
Manage care of chronically ill patients in the NICU and ambulatory settings
Assist in discharge preparation, including coordination of home medical devices such as oxygen, feeding tubes, and monitors
Provide post‑discharge follow‑up care via in‑person visits or telehealth
Collaborate with outpatient services and case management for continuity of care
Serve as a resource for the inpatient care team and follow‑up in the outpatient setting
Apply evidence‑based practice and provide patient and family education
Preferred Education/Experience Same as required: MSN for Nurse Practitioners or BS for Physician Assistants
