Responsibilities

Provide social work services to patients, families, and significant others through counseling, emotional support, environmental assistance, crisis intervention, and group leadership.

Complete a comprehensive assessment and develop a safe, realistic discharge plan of care, considering psychosocial, emotional, and financial needs.

Assess changes in the physician’s plan of care and its impact on the discharge plan.

Review the discharge plan regularly and communicate any changes to the appropriate parties; document in the electronic medical record.

Identify potential post‑discharge care problems and initiate early referrals to promote proper resource utilization and timely transfer to the appropriate level of care.

Assist the interdisciplinary team in determining alternate care methods when a patient does not require acute hospitalization and take action to minimize financial loss and improve patient care quality.

Collaborate with hospital and community resources to meet patient needs and refer to services such as income assistance, transportation, meal assistance, etc.

Assess for signs of abuse or neglect and refer to Adult Protective Services or Child Protective Services.

Assist patients in completing Living Will, Advance Directive, and Medical Power of Attorney forms.

Work with legal counsel on guardianship or conservatorship actions when necessary.

Provide education and guidance on Medicare, Medicaid, and third‑party payer coverage and regulations.

Communicate deviations from expected norms or quality of care to case management leadership, physicians, and other appropriate personnel.

Qualifications

Bachelor’s Degree in Social Work or related field.

Licensed Social Worker (LSW) or applicable state license based on location:

West Virginia and Ohio – LSW

Maryland – LBSW

Pennsylvania – LSW (state specific)

Certification in Case Management.

Two (2) years of social work experience.

Physical Requirements

Prolonged periods of standing or sitting.

Working Environment

Flexible hours, including weekends and holidays.

Skills and Abilities

Excellent verbal and written communication skills.

Collaborative work with healthcare professionals at all levels to achieve goals and improve quality outcomes.

Basic computer knowledge and ability to operate standard office software.

Schedule and Compensation

Weekly hours: 40

Shift: United States of America (Non‑Exempt)

Salary: $36,000.00–$38,000.00 annually
