Overview Social Worker MSW – Pathways Omaha Immanuel Pathways PACE® – 5755 Sorensen Parkway, Omaha, NE 68152

Schedule:

Monday through Friday, 8:00 am to 4:30 pm (flexible). Travel Reimbursement provided.

Pay:

Competitive and based on experience, certifications, skills, and education.

Responsibilities Plans, organizes, and implements social services for Immanuel Pathways participants and families, including assessment, treatment, teaching, counseling, and case management. Key duties include:

Conducting individual and family assessments.

Providing counseling and skill building to participants and caregivers.

Coordinating and delivering appropriate resources and referrals.

Advocating for participant and caregiver needs and ensuring services are met.

Managing ongoing case records and ensuring documentation accuracy.

Participating in multidisciplinary team meetings.

Assisting with participant disenrollment procedures as needed.

Qualifications & Requirements

Master's degree from an accredited school of social work.

Minimum of 2 years experience working on a multidisciplinary team in a hospital, nursing home, or community‑based setting.

Licensed or registered to practice social work in the state of Nebraska (or licensed in another state if traveling to sites where licensed).

Strong commitment to the PACE philosophy, practices, and protocols.

Ability to work independently and cover other PACE Pathways sites in the authorized state, when required.

Valid background check and drug screen clearance required prior to hire.
