This ONSITE Clinical Coordinator coordinates the daily Clinical Hospice Team functions from the office to assure effective coverage of all Team clients. Acts as a resource for field staff, including Therapists, Case Managers, Patient Care Coordinators, Nurses, Social Workers, Chaplains, and Home Health Aides, while functioning as a liaison that represents the team with all internal and external customers. Provides services at quality levels consistent with professional standards of practice. This vital position will: Ensure appropriate staffing and coordination for all team patients, and support the timely processing of all related paperwork. Provide services at quality levels consistent with professional standards of practice. Participate in holiday office coverage rotation.
