Salary Range:
$17.72-$18.52/hr
Responsibilities
The Community Habilitation Specialist displays a congenial, warm acceptance for all individuals.
The Community Habilitation Specialist will primarily be responsible for the assessment and observation of the person’s skills development. Is alert to the expressions of attitudes, feelings, and relationships of each individual. Develops an awareness of the individual’s needs through observation, interaction, case history study, and consultation with staff.
Support the person in navigating the program and identifying incentives for learning new skills and behaviors.
Engage primary caregivers and other key participants in active change-oriented treatment by identifying and overcoming barriers to engagement.
Implement problem conceptualization, treatment planning, intervention, outcome review, and strategy revision using the model's Analytic process (Brief Family Systems Therapy and Parental Management Training).
Develop Valued Outcomes with steps on how to achieve them based on caregivers/community supports and observed behaviors, along with child/adolescent strengths.
Meet with assigned children/adolescents 1-2 times per week depending on their needs and coordinated hours.
Travel to assigned boroughs as required.
Attend supervision and training sessions as required.
Know and protect individual rights.
Document each training session in the appropriate databases, reporting progress and barriers.
Receive consultation and supervision from the Community Habilitation Supervisor.
Maintain a positive and strength-focused approach to cases.
Qualifications
High School Diploma or higher (G.E.D. accepted).
Demonstrated strong commitment to safety.
