What We’re Looking For Trillium Health Resources has a career opening for an Innovations Waiver Regional Manager to join our Care Management team. This position’s primary responsibilities include overseeing a skilled, integrated care team of professionals who provide localized support within a managed care environment for members living with Intellectual or Developmental Disabilities or Traumatic Brain Injury. The Innovations Waiver Regional Manager provides oversight to ensure members receive whole person care focused on all members’ needs, including physical health, behavioral health, and social determinants of health that helps to establish positive outcomes for members and transform their lives for the better. On a typical day, you might: Provide direct supervision and administrative oversight for a localized, Integrated Care Team supporting Innovations members and members without Innovation waiver funding living with mental health, substance use, and intellectual/developmental disabilities. Ensure staff utilize workflows as required to complete Care Management Comprehensive Assessment and care plans for members as care needs change. Establish a team based, person-centered approach to coordination of care to be implemented by all team members that effectively manages a member’s physical health, behavioral health, and social determinant of health needs using established workflows and processes effectively and efficiently. Monitor teams to help ensure integration of care through the establishment of an integrated care team (e.g. member, caretaker/legal guardian, family member, PCP, behavioral health provider, specialists, nutritionists, pharmacy, etc.) to meet the member’s needs. Use knowledge and skills to create and identify methodologies and tools to be used in the execution of care management and coordination. OR Bachelor’s degree in Health, Psychology, Sociology, Social Work, Nursing, or another relevant human services area and a minimum of one (1) year of experience providing care management, case management, or care coordination to complex individuals with I/DD or TBI. OR Equivalent combination of education/experience. Preferred Experience: Supervision of two (2) or more employees preferred. Prefer experience focused on working with Innovations in the mental health, substance use, and/or intellectual/developmental disability and TBI populations. License/Certification: Must have a valid driver’s license Location: Remote within North Carolina. Must reside in NC to be considered for remote status. Must be able to cover/work in Trillium’s Southern Region of our catchment area, which includes North Carolina Counties: Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, and Scotland. Must be able to travel to a Trillium office location and within catchment as required.
