Case Management Assistant Alaska Regional Hospital, Anchorage, AK

We are seeking a Case Management Assistant to ensure high quality, efficient care for patients. As a part of the Case Management team, you will coordinate with RN case managers, social workers, and post‑acute care providers.

Job Summary & Responsibilities

Coordinate with RN case managers and social workers for patient support and updates.

Provide clerical support in multidisciplinary rounds and update NATE Tempo.

Make referrals through the web‑based system to PAC providers and send required documentation.

Monitor PAC referral patients and intervene to expedite discharge.

Prioritize workload to ensure timely contact and intervention with PAC providers.

Communicate with case managers/social workers regarding status, bed availability, and barriers.

Deliver needed letters/memos (Patient Choice, IMM, Beneficiary Notice) to patients.

Obtain physician signatures on required case management documents.

Set up transportation and generate transfer packets for PAC transfers.

Qualifications

Associate Degree preferred.

Medical knowledge and terminology required.

Minimum one year of experience in a health care setting.

Health care professional (C.N.A., E.M.T.) preferred.

Strong communication skills, both oral and written.

Analytical and critical thinking with proactive decision‑making.

Self‑motivated, goal‑directed, dependable.

Computer and typing skills.
