The Outreach and Engagement Specialist plays a vital role in building relationships with Native American individuals experiencing homelessness and connecting them to supportive services. As a frontline ambassador of NAC’s housing and behavioral health services, this role works both independently and collaboratively across departments, including Permanent Supportive Housing, Supportive Housing, Bridge Housing, Emergency Shelter, Youth Services and Behavioral Health. The ideal candidate is highly motivated, resourceful, trauma-informed, and deeply familiar with the needs of urban Native American communities as well as of the unhoused population overall. Responsibilities
Conduct community based and street outreach to locate, engage, and build rapport with target population. Meet with applicants in both an office setting and in the community throughout the Metro Phoenix area. Provide clear, accurate and comprehensive information about all NAC services Complete Housing Questionnaire assessments as needed and collaborate with other housing staff members to enter Coordinated Entry System for services. Guide clients through service navigation, helping them access housing, behavioral health, and supportive programs. Coordinate care internally within NAC and externally with partner providers, based on client goals and evolving needs. Re-engage clients when additional support is needed and facilitate linkage to appropriate services. Serve as a consistent advocate ensuring services are delivered effectively. Maintain accurate and confidential client files using an organized tracking system. Enter and update client data in the Homeless Management Information System (HMIS). Assist potential applicants with obtaining needed documents required for services. Comply with all NAC policies regarding the use of an agency vehicle. Qualifications
High School Diploma or GED required. Bachelor’s degree in social work, human services, or related field preferred. Two (2) years of experience working with individuals experiencing homelessness. Two (2) years of experience in direct service delivery or a combination of related education, training, and experience. Knowledge of the economic, social, and systemic issues impacting Native Americans communities. Strong interpersonal and communication skills, both written and verbal. Experience in working with the Native American population required. Ability to work collaboratively in an interdisciplinary team while also being self-directed and independent. Possess and maintain a valid Arizona driver’s license with a clean driving record, will be utilizing personal vehicle for outreach. Proficiency in MS and data systems, such as HMIS. Physical Demands
