· Engage individuals experiencing homelessness, mental health, or substance use challenges through direct outreach in homes, hospitals, shelters, and community settings
· Provide individual and family therapy to youth with behavioral or emotional disorders, as needed
· Assess client needs and develop individualized, recovery-oriented treatment plans
· Assist with accessing vital community resources including food, shelter, healthcare, and behavioral health services
· Provide crisis response and stabilization, ensuring safety and appropriate care coordination
· Actively collaborate with internal teams, hospitals, schools, and other providers to coordinate effective support
· Support enrollment into longer-term services such as CPRC Community Support Services
· Maintain an active caseload, completing required assessments, intakes, and documentation within deadlines
· Educate individuals and families about behavioral health, substance use, and wellness practices
· Enroll clients into applicable state systems (e.g., HMIS), and manage follow-ups and referrals
· Serve as a clinical team member in the Youth ERE Outreach Team and contribute to program meetings and strategy sessions
· Maintain up-to-date and accurate records in compliance with funding agencies and internal standards
· Participate in supervision, team development, and ongoing training
· Provide on-call coverage as needed to support the team and caseload**Education, Experience, and/or Credential Qualifications:**
· Bachelor’s degree in Psychology, Sociology, Social Work, Behavioral Health Support (BHS), or related human services field from an accredited institution
· Advanced knowledge of mental health and substance use disorders, crisis intervention, and case management practices**Additional Qualifications:**
· Must have a valid driver’s license, current auto insurance (including uninsured motorist coverage), and reliable transportation to transport clients as needed
· Must be able to respond effectively and maintain composure in crisis situations
· Must successfully complete background check including criminal record, driving record, abuse/neglect, and fingerprint check
· Must complete New Hire Orientation and meet ongoing training requirements, including Relias training annually**Physical Requirements:**
ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull, or otherwise move objects, including the human body. Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.
Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time.
