Overview

Social Worker Care Manager (Hybrid - Jersey City, NJ) Location: Jersey City, NJ. This role is primarily remote with potential in-person visits as needed. Responsibilities

Support the mental and physical health care of patients on an assigned patient caseload. Screen and assess patients for common mental health and substance use disorders; facilitate patient engagement and follow-up care. Provide patient education about mental health and substance use disorders and available treatment options. Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications. Support psychotropic medication management by consulting with the behavioral health consultant and communicating with the primary care provider. Deliver brief behavioral interventions using evidence-based techniques (e.g., behavioral activation, problem-solving treatment, motivational interviewing). Provide or facilitate referrals to evidence-based psychosocial treatments as clinically indicated. Participate in regular caseload consultation with the psychiatric consultant; share treatment recommendations with the patients medical provider. Communications may be via telephone, video, or in person. Track patient follow-up and outcomes using a registry; document encounters and use the system to re-engage patients. Document patient progress and treatment recommendations in EHR or other required systems for sharing with medical providers and other treating staff. Facilitate treatment plan changes in consultation with the medical provider and psychiatric consultant when patients are not improving or need more intensive care. Facilitate referrals for clinically indicated services outside of the organization (e.g., housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment). Develop and complete relapse prevention self-management plans for patients nearing discharge from the caseload. Be comfortable using Excel and Word and completing assessments and productivity trackers daily; manage time effectively via calendars and the EMR system. This role is primarily telephonic; in-person visits may be required. Qualifications

MSW or equivalent; minimum required degree. Licensed or provisionally licensed as appropriate. Behavioral health experience and knowledge. Licensed driver with an insured automobile in good working order. Ability to communicate effectively, both verbally and in writing. Benefits
