Job Responsibilities

Psychosocial Assessment – Performs comprehensive assessments of complex social situations using advanced interviewing skills, intuitive knowledge and experience to identify challenges and root causes, develop appropriate interventions and treatment plans. Reports suspicions of abuse and neglect to legally mandated authorities. Completes documentation with accuracy and clarity.

Resource Management – Identifies and prioritizes interventions, anticipating and eliminating barriers, facilitating problem solving and conflict resolution, and empowering patients and families to be active partners with the medical team. Partners with patients/families/caregivers to obtain financial assistance, community resources, or specialized equipment. Develops resource networks, excels at resource utilization, and acts as a resource to peers. Provides real‑time accurate information to patients/families/caregivers for eligible resources. Advocates for patient populations on a systems level by developing programs and protocols to better meet their needs. Educates patients and families on progress and adjustments in treatment plans.

Psychosocial Interventions – Provides psychosocial services as identified in the patient’s comprehensive plan of care. Creates opportunities for and provides supportive counseling to maximize emotional coping and adherence to the treatment plan. Facilitates and enhances collaboration with referral sources and the healthcare team. Uses expert knowledge to educate patients/families/caregivers and team members about evidence‑based treatment options. Acts as an expert in providing self‑management support to high‑risk/complex patients/families, developing programs, evaluating outcomes, and implementing changes based on outcomes. Motivates and empowers families through anticipatory guidance and planning to reduce or eliminate psychosocial barriers to discharge. Leads family‑centered care team meetings and demonstrates a therapeutic approach focusing on micro and macro systems, including assessment and crisis intervention to prevent problems.

Interdisciplinary Collaboration – Works to improve the system, promoting patient/family well‑being. Acts as a liaison between the patient/family/caregiver, the medical team, and outside agencies. Mediates within the medical team on behalf of patients/families/caregivers. Encourages problem‑solving and explores alternative care options to achieve desired outcomes. Develops processes for clear communication and coordination while involving patients/families in decision making to minimize fragmentation of services.

Care Transition – Partners with patients, families, and the healthcare team to ensure safe transition to the next appropriate level of care. Facilitates collaboration with referral sources, maintains knowledge of resources, and advocates for patients on a system level. Provides professional growth opportunities for others, recognized as an expert educator on psychosocial aspects of care. Assumes a leadership role in improving the effectiveness of the division, teams, and institution. Models and promotes behaviors that foster a positive work environment.

Job Qualifications

Master’s degree in Social Work or Clinical Counseling

5+ years of work experience in a related discipline

Ohio LISW or LPCC license, or the ability to transfer license to Ohio upon hire

May require additional clinical license, certification, or training
