Job Summary and Responsibilities Join our dynamic healthcare team as a Social Worker (LCSW) specializing in Care Coordination! We are seeking a compassionate and skilled professional to provide comprehensive support and advocacy for our diverse patient population. In this vital role, you will play a crucial part in navigating complex healthcare systems, ensuring seamless transitions, and connecting patients with the resources they need to thrive.

Assess patients' psychosocial needs, barriers to care, and social determinants of health through comprehensive evaluations.

Provide crisis intervention and support as needed, demonstrating sound judgment and ethical practice.

Demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her assigned unit.

Possess a strong knowledge of contemporary behavioral and systems theories relevant to health care, enabling effective assessment and intervention strategies.

Provide empathetic support for victims of abuse, neglect or violence, ensuring their safety and connecting them with appropriate protective services and resources.

Job Requirements

Master's Social Work Required

At least one year of clinical and/or medical social work experience required.

Bilingual preferred.

CA Licensed Clinical Social Worker (LCSW) required.

Fire Safety Card within 90 days of hire is required.
