Case Planner – Foster Program

As a Case Planner, you'll be a powerful advocate for children and families, guiding them through life's most challenging transitions and helping build brighter futures. You'll work closely with foster parents, biological families, and service providers to ensure each child receives the support they need to thrive.
Responsibilities
Manage a caseload of children in foster care, ensuring their safety and well-being.
Develop and implement individualized treatment plans focused on permanency and stability.
Support foster parents with parenting strategies and behavioral guidance.
Maintain consistent communication with families, schools, and service providers.
Represent cases in Family Court and ensure every child's voice is heard.
Document progress and maintain compliance with agency and ACS standards.
Requirements
Bachelor of Arts/Arts or BSW and 2 years of Human Services experience required.
Work experience in Child Welfare and/or Office of Mental Health System preferred.
Computer literacy required.
NYS driver's license preferred.
Bilingual in Spanish preferred.
