MDS Coordinator - Open to RN and LPN Candidates. Part Time Position - Flexible Schedule! This position is onsite in the Raleigh, NC area. Assist the Director of Nursing with ensuring documentation in the facility meets Federal, State, and Certification guidelines. Coordinate the RAI process to assure timeliness and completeness of the MDS, CAAs, and Interdisciplinary Care Plan. Assure completion of the RAI Process from the MDS through the interdisciplinary completion of the plan of care. Initiate and monitor RAI process tracking, discharge/reentry, and Medicaid tracking forms through the EMR system. Follow up with staff to ensure compliance with documentation standards. Complete patient assessments, data collection, and interview staff as necessary to assure quality practice per the current version of the RAI Manual and company policies. Facilitate accurate determination of the Assessment Reference Date that reflects patient care needs and resources used for appropriate payment by Medicare/Medicaid and insurance programs. Provide an interdisciplinary schedule for all MDS assessments and care plans as required by OBRA and PPS. Ensure appropriate signatures are obtained as required. Support decisions about completing additional MDS assessments based on clinical criteria per the latest RAI User’s Manual (Significant Change and Significant Correction). Assist with identification of significant changes, physician orders, and verbal reports to ensure MDS and care plans reflect changes. Coordinate scheduling and notice of patient care planning conferences, and communicate outcomes/problems to the responsible staff, patient, and/or responsible party. Assist with ensuring all MDS information and care delivered is supported by documentation per the Care Plan. Assist the Executive Director/Director of Health Services in initiating a care plan for every patient upon admission to the center. Participate in daily stand-up meetings and communicate needs for changes in PPS timelines, Assessment Reference Dates, and deficiencies in MDS, CAAs, and Care Plans. Relay or act upon information from Case-Mix Specialist audits. Serve as a resource for computer issues related to the MDS process and contact the help desk when indicated. Ensure appropriate Medicare coverage through regular communications with consultants. Attend daily and weekly Medicare or Utilization Review meetings as required. Correct and ensure completion of final MDS and submit patient assessment data to the appropriate State and Federal government agencies. Assign, assist, and instruct staff in the RAI Process, PPS Medicare, Medicaid (Case Mix as required) and related clinical computer systems. Ensure timely submission of MDSs to the State with proper follow-up on validation errors; maintain validation records in a systematic and orderly fashion. Education

Graduate of an approved Registered Nurse program and licensed in the state of practice required. Experience and Qualifications

Ideal candidate will have RN or LPN nursing experience in a Skilled Nursing Facility. Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement; thorough understanding of PDPM. Knowledge of OBRA regulations and Minimum Data Set (MDS); knowledge of the care planning process. Minimum of one year of MDS experience. Job Details
