A Brief Overview
Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves a resource to others in work-team and department.
What You Will Do
Provides psychosocial assessment and intervention to assist patients and families, including clinically complex situations.
Provides age‑specific care to patients and families in all age groups.
Develops, documents and implements a comprehensive plan based on identified psychosocial and/or discharge needs. Develops plans with attention to individual patient and family goals.
Provides linkage to community resources and UH network services to facilitate optimal care.
Communicates and collaborates with the work team using a multidisciplinary approach.
Provides guidance and consultation to peers and team regarding routine psychosocial issues and/or discharge planning issues.
Provides supervision to social work interns if LISW eligible and completes required supervision hours.
Additional Responsibilities
Performs other duties as assigned.
Complies with all policies and standards.

Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
