The Supportive Housing Case Worker provides ongoing case management services to individuals and families recovering from substance use disorder. Services are delivered in the office, clients' homes, or in the community to support recovery and housing stability. Responsibilities
Establish positive professional relationships with clients. Support and coordinate efforts to maintain housing stability. Conduct intakes and assessments for case management. Assist clients in accessing benefits such as SSI, SSDI, SNAP, TANF. Link clients to community resources to promote economic independence. Teach skills like transportation, budgeting, and daily living activities. Provide training on life skills including financial literacy, nutrition, and parenting. Advocate for clients and teach self-advocacy skills. Develop and monitor individual service plans with SMART goals. Complete reports and tracking documentation. Willingness to drive agency vehicle if required. Respond to after-hours calls and emergencies. Perform additional tasks as assigned by management. Engage in community trainings and agency meetings. Respect cultural and linguistic diversity in all interactions. Minimum Qualifications
Ability to work with diverse populations. Lived experience preferred. Valid driver’s license and willingness to use own vehicle. Bi-lingual/bicultural applicants encouraged. Commitment to racial equity and inclusive practices. Knowledge of community resources and services. Ability to provide client-centered services.
