Summary The provision of these job responsibilities are mandated by the Centers for Medicare & Medicaid (CMS) for Medicare-approved transplant centers. As a member of the transplant team, the Solid Organ Transplant (SOT) Social Worker utilizes specialized clinical knowledge and advanced clinical skills to help in the areas of assessment, treatment of medical, patient and caregiver navigation, mental health and behavioral disorders, and discharge planning.

Responsibilities

Conduct a detailed psychosocial assessment to determine candidacy for solid organ transplant or living donation.

Participate in discharge planning, including coordinating patient transfer to other facilities.

Monitor progress toward goals of the social work plan of care and revise it in response to changes in patient needs and condition.

Organize and facilitate care conferences with patients, families, representatives, and multidisciplinary care team.

Ensure attendance at multidisciplinary rounds or huddles and report on psychosocial status and its implications for transitions of care.

Participate in the Transplant Selection Committee and present the mental health, social status, and functional abilities of candidates.

Education and Certification Requirements Master’s: Social Work (Required). Licensed Clinical Social Worker (SW License) – State of Florida (FL).

Additional Job Information Complexity of Work: Requires critical thinking, effective communication, decisive judgment, positive relationship building; must lead others and take appropriate action. Requires strong organizational, priority setting and multi-tasking skills; ability to learn and apply large amounts of technical and procedural information.

Required Work Experience: Three (3) years of social work experience. Transplant experience preferred.

Must be licensed as a Florida board-certified Clinical Social Worker (LCSW).
