Social Worker-Case Management CONTINGENT role at Henry Ford Health. CONTINGENT, ROTATING SHIFTS, DAYS. Responsibilities
Coordinates the overall interdisciplinary plan of care for the patient, from admission to discharge. Creates plan for care across the continuum, integrating patient/family preferences and values. Serves as a resource to assess, evaluate, recommend and treat legal, neglect, substance abuse, crisis, psychosocial and various adjustment needs. Advocates for resources and removal of barriers. Maintains ongoing dialogue with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan. Acts as a resource for adequate medical record documentation, complex level of care recommendations, and services as they relate to diagnoses and treatment options for post-discharge care. Licensure / Certification / Registration
One or more of the following required: Counselor credentialed from the Michigan Board of Counseling; Social Worker credentialed from the Michigan Board of Social Work (any level); or Case Manager credential from the Commission for Case Manager Certification (CCMC) preferred. Education
Bachelor's degree required. Master's degree preferred. Experience / Other
Behavioral Health experience preferred. Additional Information
