Description As a member of the Hudson Valley Hospice Interdisciplinary Team, the Social Worker’s primary focus is supporting patients, families, and staff through end‑of‑life issues, decision‑making, and anticipatory grief. The Social Worker provides emotional support and counseling, delivers concrete social services as needed, and actively participates in developing the patient‑family care plan.

What you will do:

Facilitate the patient’s and family’s understanding of the demands and challenges of terminal illness, dying, death, and grief through ongoing assessment and intervention, including individual and group counseling, grief work, family meetings, and crisis intervention.

Provide crisis intervention as needed.

Implement concrete services and coordinate with allied help, social service agencies, and institutions on behalf of hospice patients/families, ensuring appropriate follow‑up and advocacy.

Maintain knowledge of community services and nursing home placement processes and make timely referrals.

Assist with Medicaid and MLTC application processes related to community and institutional benefits, helping families with applications as indicated.

Provide in‑service and education to patients, families, staff, and the community.

Collaborate with the Interdisciplinary Team to identify bereavement risk factors before a death event and during the transition to Bereavement Services.

Complete a comprehensive psychosocial assessment of the patient and family within five days of admission.

Assist patients and families with advanced directives (MOLST, Health Care Proxy, etc.) and coordinate funeral planning information.

Plan for the patient’s discharge and custodial care, ensuring all support and resources are in place and develop comprehensive care plans addressing physical, emotional, and social needs during end‑of‑life care.

Demonstrated communication skills and ability to facilitate communication between family members and patients.

Ability to recognize families’ strengths and weaknesses, access extended support systems, and be sensitive to ethnic, cultural, and lifestyle differences.

Effective verbal and written communication skills.

Capability to prioritize and handle multiple tasks efficiently.

Excellent attention to detail.

Work Environment

Work in assigned settings including patient/family homes, hospitals, and nursing facilities throughout Ulster or Dutchess counties.

Moderate travel by car.

Routine duties may involve direct exposure to blood or bodily fluids.

Education & Experience

Master’s degree in social work.

Valid New York State Social Work License or Limited Permit (LCSW preferred).

Minimum of two years of social work experience with at least one year in medical social work, grief counseling, hospice, or community health preferred.

Required experience working in the healthcare field.

Knowledge of family and group treatment dynamics related to death, dying, and grief preferred.

Excellent counseling, advocacy, and collaboration skills.

Valid New York State driver’s license with reliable transportation.

Hours Full Time, 40 hours/week (Monday‑Friday 8:30 am‑4:30 pm); On‑call rotation required.

Pay $65,000 – $66,300 annually.

FLSA Exempt.
