Overview
Social Worker position based in a hospital setting. The role collaborates with physicians, staff and other health care professionals to coordinate care for selected patient populations across the continuum within the acute care setting. The Social Worker works with patients and families to meet psychosocial and transitional needs and participates in system-wide quality improvement initiatives as part of the Care Management team. Essential Functions
Conducts psychosocial assessments on identified priority patients in collaboration with family. Outline available strategies and services to address psychosocial care needs and provide recommendations as appropriate. Utilizes specialized screening tools (e.g., Depression, Anxiety, Substance Abuse, Health Literacy, Mental Status, Social Support) and other evaluative tools as needed. Develops individualized psychosocial case plans in collaboration with the Case Manager – Acute Care and Care Management team. Documents actions in the medical record per departmental guidelines and coordinates information exchange with other facilities/agencies while adhering to confidentiality requirements. Provides crisis interventions and psychosocial counseling to support patients and families’ social and psychological health, using available resources. Provides factual information and materials to patients/families regarding programs for end-stage illnesses and other services; makes referrals to appropriate agencies when needed. Screens for financial assistance and clarifies insurance benefits; supports patients and families in managing financial stress associated with serious illness; assists in enrollment in needed programs and services. Assesses service needs through multidisciplinary rounds and collaboration with physicians and other interdisciplinary team members. Consults with outside agencies to meet identified goals and coordinates a broad spectrum of discharge planning services during hospitalization. Collaborates with patient/family, physicians, and care management staff to determine post-discharge needs and plans; provides information on options/resources and assists with referrals. Note:
