We’re seeking a full-time Hospital Clinical Liaison to serve as the primary point of contact for collaborative services with hospital partners, acting to enhance continuity of care for shared clients. As a clinical provider, the Hospital Clinical Liaison will provide on-site mental health assessments to newly referred clients and secure mental health services post-discharge as appropriate. Additionally, the Hospital Clinical Liaison will work collaboratively with hospital staff to establish relationships, workflows, and systems for effective communication. The ideal candidate will have: Must be actively licensed by the State of Ohio Counselor, Social Worker and Marriage and Family Therapist Board. Prior experience in administering diagnostic assessments and utilizing various mental health-screening tools strongly desired. Must have an awareness of and sensitivity to the cultural diversity of the populations served. Must have a valid driver’s license in the State of Ohio and access to an insured vehicle.
