Position Summary The ERF Case Manager plays a vital role in stabilizing homeless individuals and families by providing intensive case management, housing navigation, and wrap‑around support services. Working under the supervision of the Program Director, this position connects clients experiencing housing instability to safe, stable housing and supportive services that promote long‑term self‑sufficiency.
Role Description This is a full‑time, on‑site role located in Redlands, CA. The ERF Case Manager will be responsible for assessing client needs, developing and implementing case plans, coordinating services, providing resources, and offering support to ensure clients achieve their goals. The ERF Case Manager will also document all client interactions, maintain accurate records, and assist clients in securing basic needs and housing stability.
Key Responsibilities
Conduct intake assessments to determine housing needs, eligibility, and service plans for individuals and families experiencing homelessness.
Develop individualized housing stability plans addressing housing, income, health, and social service needs.
Assist clients in securing permanent housing—locating units, negotiating with landlords, and completing documentation.
Provide ongoing case management after housing placement to ensure retention and continued progress toward stability.
Collaborate with the City of Redlands, local shelters, housing authorities, landlords, and service providers to coordinate referrals and resources.
Support clients in accessing benefits, employment services, behavioral health resources, transportation, and other supports.
Conduct home visits, community outreach, and regular follow‑ups to maintain engagement and monitor housing stability.
Maintain accurate and timely case notes, data entry, and documentation in compliance with agency and funding requirements.
Advocate for clients to reduce barriers and increase access to supportive services.
Participate in regular staff meetings, training, case conferences, and professional development activities.
Qualifications
Bachelor’s degree in Social Work, Human Services, Psychology, or related field (Master’s preferred). Equivalent relevant experience may substitute for education.
Minimum 2 years of experience in case management, homeless services, or housing navigation (nonprofit or social services experience preferred).
Strong understanding of housing systems, landlord/tenant law, and supportive housing programs (e.g., Rapid Rehousing, PSH).
Knowledge of trauma‑informed care, harm reduction, and motivational interviewing practices.
Excellent communication, problem‑solving, and crisis intervention skills.
Ability to work independently, manage multiple priorities, and meet deadlines.
Proficiency in Microsoft Office; experience with HMIS (Homeless Management Information System) preferred.
Valid driver’s license, reliable transportation, and ability to travel locally as required.
Bilingual (English/Spanish) preferred.
Work Environment & Physical Requirements
This position involves both office and fieldwork, including home visits and community‑based meetings.
Must be able to sit, stand, and move throughout the workday; lift up to 25 pounds as needed.
Occasional evening or weekend hours may be required to accommodate client needs.
Fingerprint and drug screen clearance, along with a current negative TB test, are required.
Must maintain a valid driver’s license and proof of current vehicle insurance.
Additional Information This is a 100% grant‑funded position. Continued employment is contingent upon the availability of funding through secured grants on an annual basis.
