Overview
Emergency Medicine Clinical Pharmacy Specialist – Bayhealth The Emergency Department Clinical Pharmacy is responsible for the provision of comprehensive clinical pharmacy services for the emergency department. This includes pharmacokinetic and therapeutic consultations, code and trauma participation, and nursing education for new therapies. Base pay range: $136,406.00/yr - $211,411.00/yr. Location: Sussex Campus Hospital, Dover, DE; status: Full Time; shift: Variable. Responsibilities
Conduct staff development programs or educational services to pharmacy staff, pharmacy students and residents, nursing staff, patient care services, physicians, medical residents, and other healthcare professionals Serve as a subject matter expert in emergency medicine, completing drug information questions for the emergency department Provide comprehensive pharmacy review of emergency department medication orders including medical emergencies, code resuscitation efforts, and high-risk procedures. Reviews include order verification, indication, dosing, allergy assessment, organ function, IV to PO, antimicrobial de-escalation and drug interactions Provide patient-specific medication education using teach-back method for discharge medications when appropriate Act as an integrated member of the ED multidisciplinary team, participating in patient care decisions including bedside management of medical emergencies (trauma, stroke, psychiatric, toxicology), resuscitations, transition of care, pain management, assisting with opioid use disorder, and discharge medication reconciliation Identify and evaluate medication-related problems based on clinical presentation, available history, or laboratory data Review antimicrobial cultures and perform patient antibiotic call-backs for patients discharged untreated or on inappropriate empiric antibiotics; follow-up for sexually transmitted diseases identified by the Delaware Public Health Department Review Emergency Department medication error reports to identify trends and promote safe medication use; perform quality assurance activities Serve as a preceptor to pharmacy residents and students Participate in Emergency Department meetings, education and training Promote rational, cost-effective drug therapy focusing on cost avoidance and savings Establish relationships with local, state, and national organizations to stay informed and participate in initiatives All other duties as assigned within the scope of job responsibilities Required Education, Credential(s) and Experience
Education: Bachelor Degree in Pharmacy; additional BS, BPS Pharmacy Board Certification, and certificate in Emergency Medicine or Critical Care Credential(s): Licensed Pharmacist; Basic Life Support; Advanced Cardiovascular Life Support; Pediatric Advanced Life Support; eligible for licensure in Delaware Experience: Two years of hospital experience and completion of clinical training Preferred Education, Credential(s) and Experience
Education: PharmD; PGY-2 residency in Critical or Emergency Medicine or board certification in Critical Care Pharmacy or Emergency Medicine Pharmacy Experience: PGY-2 ASHP-accredited residency in Critical or Emergency Medicine or PGY-1 ASHP-accredited residency with minimum 3 years related experience and BPS board certification; or minimum 5 years experience in Critical or Emergency Medicine with relevant BPS certification Location
