Overview
Licensed Clinical Social Worker (LCSW) – Medical Respite at Central Health. This role is part of Behavioral Health Services (LCSW-BHS) and focuses on providing compassionate, inclusive, trauma-informed clinical care to reduce barriers to mental health and substance use supports, treatment, and services. The LCSW-Medical Respite serves Medical Respite patients and works out of the Medical Respite facility. Reports to the Behavioral Health Manager. Responsibilities
Conduct diagnostic assessments of mental health and/or substance use disorders using the DSM and other clinical assessments and screenings to deliver evidence-informed care addressing biopsychosocial-spiritual needs, strengths, and goals. Provide trauma-informed, evidence-based, strengths-based, and culturally affirming therapeutic counseling for individuals, in collaboration with the patient’s treatment team and care providers, and develop care plans aligned with Central Health’s policies and goals. Lead and facilitate psychoeducational and counseling groups; foster group cohesion, encourage participation, and manage group dynamics. Foster a safe, supportive, and healing environment for patients and identified support systems; serve as a subject matter expert on social work approaches, interventions, and treatment of mental health and substance use disorders. Respond to mental health and substance use crises with trauma-informed crisis intervention, de-escalation, risk assessment, and other evidence-informed interventions, independently or with community partners. Advocate with internal and external stakeholders for patient-centered choices, values, and preferences; maintain knowledge of community resources to refer patients for services. Collaborate with the patient’s treatment team to ensure care aligns with Central Health’s policies, mission, and goals. Support continuous quality improvement and utilization management initiatives to ensure high-quality patient care. Attend and participate in scheduled staff meetings, supervision, training, and other relevant meetings for clinical, administrative, and operational functions. Complete all direct and indirect patient care documentation in accordance with professional standards and relevant regulations. Provide mental health education for staff to support trauma-informed care and behavioral health services. Perform other duties as assigned. Qualifications
Minimum Education:
Master’s Degree in Social Work from an accredited school of Social Work Minimum Experience:
At least 2 years of professional experience performing mental health and substance use assessments, diagnosis, and individual/group treatment Required Certifications/Licensure:
