Patient Navigator (RN) – Hematology Galveston, Texas, United States Nursing & Care Management – UTMB Health Job Summary
The Patient Navigator guides patients through the healthcare process, ensuring that the healthcare system meets the needs of the patient as best as possible. The navigator communicates with patients and families regarding procedures, preparation, recovery, follow-up, and refers physicians. The navigator reduces fear about procedures by explaining expectations and answering questions. Minimum Qualifications
Bachelor’s degree in Nursing Three (3) years of previous clinical RN nursing or RN patient navigator experience Licenses, Registrations or Certifications Required
BLS Current RN license or valid permit to practice professional nursing in Texas Certification in nursing specialty Preferred Qualifications
Oncology experience is highly preferred. Job Duties
Navigates patients through diagnostic evaluation, educates, supports, and empowers patients to make informed treatment decisions, remaining a support system throughout treatment. Collaborates with patients, families, and providers in decision‑making, offering medical guidance and guiding patients and caregivers through the treatment process. Provides consultation and coordination throughout diagnosis, treatment, and follow‑up, communicating with patients to schedule procedures, providing preparation instructions, and following up with referring physicians on procedure status. Maintains open lines of communication to answer all questions prior to or after procedures, using clinical expertise to make recommendations. Identifies and develops relationships with departments involved in patient care, educating them on the patient navigator role and facilitating interaction and communication with all health‑care staff involved in procedures. Salary Range
Actual salary commensurate with experience. Work Schedule
On‑site, Monday through Friday, 8:00 a.m. to 5:00 p.m., and as needed on occasions. Equal Employment Opportunity
