Overview Harbor is currently looking to add a Crisis Clinician - LSW/LPC, LISW/LPCC, LISW-S/LPCC-S to the Urgent Care/Assessment team in Toledo. Position is per diem. Must be available for every other weekend work and some holidays.

Responsibilities

Assist in providing access to care for clients.

Act as a key provider in initial Diagnostic Assessment of new, ongoing or crisis clients.

Complete discharge or linkage to other services as needed.

Provide diagnostic assessments, treatment planning and updates as required.

Type clinical documentation in Harbor’s approved format and submit all documentation according to Harbor policy.

Provide crisis intervention services and be available for consultation.

Facilitate referrals to internal and/or external resources as indicated.

Manage caseload effectively and ensure timely transfers/discharges; open inactive cases should not exceed 10% of total caseload.

Provide crisis assessments, stabilization, and service coordination to de-escalate crisis and promote safety.

Qualifications and Requirements

Requires a Bachelor’s degree or Master’s degree in Counseling, Social Work or a related Behavioral/Mental health field from an accredited college or university.

Experience with youth and adult substance use assessment is a plus.

Requires license to practice as a Social Worker/Counselor in Ohio (LSW/LPC, LPCC/LISW).

Must be able to communicate clearly, verbally and in writing, with a variety of audiences, and be willing to represent the organization within the medical/health-care community.

Must be proficient and accurate in computer use.

Must have good documentation, treatment and assessment skills.

Prefer licensure in chemical dependency or proven experience and training in treating substance use disorders.

Must have valid Driver’s license, acceptable driving record, and be deemed insurable through Harbor’s vehicle insurance carrier.

CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.

Crisis Program The Crisis Program provides crisis intervention when clients lack coping skills and face barriers such as substance use, medication access, or unmet basic needs. It emphasizes safety planning and, when necessary, hospitalization. With Harbor’s Urgent Care opening, ambulatory detoxification, emergency medication, and resources will support clients in meeting their basic needs.

The ideal Crisis Clinician is patient, adaptable, able to work with ambiguity, and able to stay calm in challenging situations while exploring options of care to help clients and families achieve their goals.
