Job Summary: Under the direct supervision of the CCO and the Recovery Services Manager, the Case Manager provides a variety of relationship-based interventions, including group counseling, crisis intervention, individual counseling sessions, staff coaching and training, admissions support, and discharge planning oversight, along with other relevant services to clients and the company. The Case Manager is a clinical team leader offering comprehensive social, interpersonal, counseling, and recovery services for clients experiencing SUD and MH concerns and life consequences.
Minimum Qualifications: Education: CADAC Certification I/II; Bachelor's Degree (Preferred)
Experience: Addiction Counseling - 3 years (regular); Mental Health - 3 years (regular)
License/Certification: BBS, CCAPP, CAADE, CADTP (Required)

Essential Duties and Responsibilities: Under supervision of the CCO and Program Director, the Case Manager provides individual and group counseling, crisis intervention, and other services. Works as part of an interdisciplinary team in a primary care setting. Must be certified as a CAADE, CADTP, or CCAPP. Demonstrates ability to create and maintain boundaries, complete documentation on electronic records (KIPU), facilitate groups, manage multiple deadlines, work independently, and handle sensitive matters confidentially. Works effectively with diverse populations.
Core Duties: Relate compassionately to clients with various needs
Encourage clients to follow care plans
Communicate effectively with clients, families, and professionals
Assess clients' wellness and develop tailored care plans
Coordinate with support networks and health professionals
Record and evaluate client progress
Follow up post-discharge
Manage high-paced treatment environment
Assist clients with long-term planning
Demonstrate leadership and cultural competence
Work flexible hours, including evenings
Ensure HIPAA compliance
