The ambulatory Pharmacy Navigator program helps HMH Discharge and Specialty Pharmacy patients receive their discharge/specialty medications quickly and efficiently. The Navigator I ensures patients receive their prescriptions smoothly by gathering information, prioritizing tasks, addressing financial/insurance/clinical challenges, and delivering prescriptions before discharge or to patients at home. The Navigator I functions include all job functions of an Ambulatory Pharmacy Technician plus additional duties described below. Responsibilities
The Navigator duties include all job functions of an Ambulatory Pharmacy Technician plus additional duties. Establish an initial greeting to patients admitted or with a clinic appointment and explain services available via the HMH Pharmacies. Perform prescription order entry duties as necessary. Obtain patient authorization to process prescriptions through the ambulatory or specialty pharmacy. Collaborate with floor nurses, discharge planners, case managers, social workers, physicians, APNs, PAs, Medical Residents, unit clerks and other teams to ensure timely medications for patients scheduled for discharge or clinic appointments. Determine information sources necessary to process prescriptions (e.g., insurance coverage). Identify and troubleshoot patient medication access issues related to insurance and finances and help resolve so patients receive needed medications. Adhere to established HMH Pharmacy and Federal/State privacy and compliance policies to protect patient confidentiality. Ensure prescription fulfillment by in-house pharmacy to support timely discharge and/or start of specialty medication. Coordinate and facilitate patient pickup of prescriptions and/or delivery to patient care areas or home. Ensure offer for pharmacist counseling and provide pharmacist contact information for prescription-related questions at home. Facilitate distribution of educational materials and coordinate counseling with staff pharmacist as needed. Assist Ambulatory Pharmacy staff as needed; perform duties of Ambulatory Pharmacy Technician if required. Maintain a safety-first ethic and report any medication errors or variances per HMH policy. Perform other duties and/or projects as assigned. Adhere to HMH organizational competencies and standards of behavior. Qualifications
Education, Knowledge, Skills and Abilities Required
High School diploma, GED, or equivalent programs. Excellent written and verbal communication skills. Proficient computer skills including Microsoft Office and/or Google Suite. Education, Knowledge, Skills And Abilities Preferred
Retail/Specialty Pharmacy experience. Strong knowledge and experience with PBM Claims and coverage. Licenses And Certifications Required
Pharmacy Technician License. PTCB or equivalent certification required within 180 days of start, with exceptions for certain hires. Compensation: Starting at $25.38 Hourly. The posted rate is a good faith estimate of the minimum base pay and may vary with market and other factors.
