About the Position
Under general supervision of the Chief Impact & Operations Officer, the Case Manager will work with YWCA shelter clients and Norfolk Family Justice Clients. The Case Manager is responsible for meeting with clients/guests to conduct an initial assessment of need, connect clients to appropriate services and maintain a caseload to support and coordinate services for shelter guests and community clients. Case Managers will work collaboratively with the Housing Case Manager, Housing Advocates, Victim Services and Allied Professionals. Essential Duties
The essential functions include, but are not limited to the following: Conduct initial service intake and needs assessments to determine client needs and coordinate services based on intake assessment. Complete thorough assessments in order to identify strengths, needs barriers, and goals. Completes scheduled check ins with clients on caseload to provide support, information, referrals and goal progression tracking. Provide education regarding the dynamics of sexual assault, domestic violence, stalking, and human trafficking to victims of interpersonal violence. Provide education on reporting options, protective orders, and the effects of trauma on victims of interpersonal violence. Provide in-person crisis intervention counseling and advocacy service, including safety planning and suicidal assessments. Act as liaison between clients and Norfolk Family Justice Center community partners. Provide ongoing case management services to community clients or clients exiting shelter. Provide information and community referrals as requested. Collect statistical data and other information needed for grant reporting. Create and accept internal and external referrals for resources and services and maintain communications with providers. Collaborate with support staff to mutually inform teams on current guest/client issues and/ or needs. Assist shelter guests with housing applications and search (when applicable) and refer to Housing Specialist if appropriate. Maintain client case records by the YWCA SHR policy and procedures. Maintain client confidentiality by the YWCA SHR’s Confidentiality policy. Adhere to the YWCA SHR’s Mandated Reporter policy in collaboration with law enforcement, forensic and medical personnel, and other allied professionals. Participate in on-going training and education regarding interpersonal violence, community resources, and medical personnel. Prepares and delivers educational programs to allied professionals, community members, and volunteers. Attend staff meetings as scheduled throughout the year. Participates in fund- raising and development activities that support the organization. Performs other duties as assigned. Metric and Key Performance Indicators
85% of clients who conduct an intake assessment will compile a safety plan. 80% of clients requesting case management services will receive two or more referrals. Qualifications

Education and Experience A bachelor’s degree in social work, psychology, criminal justice, human services, or a related field, or eight years of related experience is required. Experience working with victims of interpersonal violence, persons with disabilities, mental health concerns, and chronic homelessness is required. Other Requirements Must complete a background check. Must complete the training program of the YWCA SHR. Must have excellent driving record and have a valid driver’s license; certificate of insurance from an insurance carrier licensed in the State of Virginia. Travel is required and access to reliable transportation is essential. Skills and Competencies Knowledge of the dynamics of interpersonal violence and a strong commitment to ending violence against women, men, and children. Demonstrate the ability to work well with people of various ethnic, racial, and socioeconomic backgrounds. Demonstrate the ability to communicate clearly and effectively both verbally and in writing. Proficiency with Microsoft Office applications, including spreadsheets. Must be a self-starter with the ability to work independently and as a part of a team. Effectively manage multiple priorities. Excellent organizational and interpersonal skills. Demonstrate the ability to maintain a calm and supportive demeanor at all times.
