Essential Duties and Responsibilities

Explain procedures and treatments to individuals to obtain cooperation and understanding.

Administer prescribed medication and treatments according to MAP Guidelines.

Maintain awareness of comfort and safety needs of individuals.

Observe, record significant conditions, reactions, and notify the supervisor or physician.

Take temperature, pulse, blood pressure, and other vital signs.

Respond to life‑saving situations based on nursing standards, policies, procedures, and protocol.

Assist staff as needed to provide care, welfare, safety and security to clients.

Oversee medication administration within the program per DDS regulations.

Act as liaison between DDS, Day Program, and families regarding medical care.

Complete/update medical assessments annually or per manager’s request.

Maintain monthly progress notes for each participant.

Assist and support individuals with personal care responsibilities (diet, exercise, meal prep, bathing, toileting).

Assist with physical direct care including mechanical transfers from wheelchairs or beds.

Conduct individualized medical training for residential staff.

Participate in on‑the‑job training and coach new co‑workers as directed.

Perform additional duties or substitute for others as required.

Adhere to all RTR policies and procedures.

Qualifications

Current LPN license.

Minimum of one year related experience.

Valid driver’s license.

Basic knowledge of Microsoft Word and the Internet.

Successful completion of all state and agency mandated training, including medication administration rules and regulations.

Must be willing to authorize a Criminal History Background Check (CORI) and release of information to DPPC, DDS, and other agencies.

Language Skills Must be fluent in English and able to read and interpret professional journals, technical procedures, and governmental regulations. Ability to write reports, business correspondence, and procedure manuals, and to effectively present information and respond to questions from managers, clients, customers, and the general public.

The employee is occasionally required to climb or balance. The employee must frequently lift and/or move up to 25 lbs and occasionally lift and/or move up to 100 lbs. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

Reasoning Ability Ability to apply common sense understanding to carry out detailed written or oral instructions.
