We are dedicated to providing patients and their families with hospice care rooted in the communities we serve. Our Ohio-based management team, through our local clinicians, responds to patient needs in a timely and appropriate manner because we are local. This position supports collaboration with the Hospice Liaison and Admissions teams to increase public awareness of hospice services and to drive admissions for patient placement in line with the plan of care and company policy. Schedule: 40 hrs/week. We are committed to providing our employees the tools to do their jobs effectively and deliver the best hospice care in the Dayton, OH area. We don’t require high caseloads or unmanageable quotas, which allows our team more time with each patient. Our top priority is our patients and the people serving them.

Responsibilities

Collaborate with the Hospice Liaison team and Admissions team to increase public awareness of hospice and other agency services and to drive admissions for patient placement to meet expectations. Act as the first point of contact for hospitals, health care facilities, and other sources regarding referrals. Seek networking opportunities to educate and develop relationships with new and existing referral sources. Identify appropriate care and services for patients and families in accordance with the attending physician and plan of care, and company policy and procedures.

Position Requirements

Bachelor’s degree in marketing, business administration, or related fields is preferred. An acceptable combination of experience and/or training may be considered instead of formal education. Licensed as a registered nurse in the state of practice, a plus Must possess the ability to make independent decisions when circumstances warrant Must possess the ability to deal tactfully with patients, family members, visitors, agency personnel, and the general public Must be knowledgeable of quality assessment and assurance procedures Valid driver’s license and proof of insurance is required
