Job Summary The Chronic Registered Nurse provides and coordinates clinically competent care for patients with End Stage Kidney Disease (ESKD). This includes assessment, planning, intervention, and evaluation of assigned patients, as well as delegation of care tasks to RNs, LPNs/LVNs, and Patient Care Technicians. The RN plays a key role in ensuring safe, high-quality dialysis treatment, patient education, and continuous improvement of clinical outcomes within the facility.
Key Responsibilities
Perform comprehensive patient assessments before, during, and after hemodialysis treatments and document all care activities.
Collaborate with interdisciplinary care teams to plan, evaluate, and implement individualized care plans.
Administer medications as prescribed and document medical justification and treatment effectiveness.
Respond to medical emergencies and assist in implementing emergency measures when needed.
Provide education and support to patients and families regarding dialysis, transplant options, and treatment plans.
Supervise and mentor Patient Care Technicians, LPNs/LVNs, and new nursing staff as assigned.
Ensure correct laboratory specimen collection, processing, and documentation.
Participate in quality improvement initiatives and maintain compliance with all state, federal, and organizational regulations.
Serve as a resource for staff on clinical procedures and documentation standards.
Physical Demands & Work Environment
Direct patient care involving frequent standing, bending, and lifting.
Must be able to assist patients with ambulation and move equipment weighing up to 200 lbs. (with assistance).
Exposure to infectious materials in a healthcare facility environment.
May require travel for training or coverage at nearby facilities.
Color vision testing (Ishihara Test) is required as part of employment screening.
Supervision
Provides oversight of Patient Care Technicians, LPNs/LVNs, and RNs as Team Leader after meeting all training and experience requirements.
Requires at least 9 months of RN experience and 3 months of chronic or acute hemodialysis experience.
Education and Licensure
Graduate of an accredited School of Nursing.
Current State RN licensure.
Current CPR/BLS Certification.
Must meet all practice and licensure requirements for the state of Virginia.
Required Experience and Skills
Minimum 1 year of nephrology nursing experience in the past two years as an RN.
Chronic hemodialysis experience required.
Excellent communication and leadership skills.
Strong clinical judgment, attention to detail, and ability to work effectively in a team-based environment.
