Licensed Practical Nurse (LPN) – Congestive Heart Failure Clinic – St. Joseph
Join Bon Secours Mercy Health in our Congestive Heart Failure Clinic as a Lead Licensed Practical Nurse. The role supports the Practice Manager, coordinates practice personnel, and provides comprehensive nursing care to patients and families under physician direction.
Responsibilities
Analyze assessment data, evaluate patient condition, and collaborate with providers to develop and maintain patient care plans.
Provide education to patients, families, and staff; collaborate with other healthcare entities under provider or manager direction.
Coordinate patient care across healthcare entities according to the patient’s plan of care.
Administer medications safely in compliance with organization policies, procedures, and state licensing requirements.
Serve as subject matter expert and mentor within the practice.

Required Qualifications
Active state Licensed Practical Nurse (LPN) licensure.
Basic Life Support (BLS) certification.
Preferred Qualifications
One year of clinical patient care experience (preferred).
Training
EPIC Electronic Health Record (EHR) training (preferred).
Skills
Demonstrated knowledge and skills to provide appropriate care to patients of all ages.
Proficiency with computer‑based appointment scheduling and registration systems.
Ability to work in a fast‑paced team environment.
Strong interpersonal communication and organization skills.
Accurate vital sign documentation.
Patient rooming and monitoring during office treatments.
Accurate medical record documentation.
Medication and vaccination administration within scope.
Effective communication with medical professionals and family members.
Management of patient messages and communications.
Scheduling of tests and appointments.
Telephonic patient triage.
Attention to detail, critical thinking, acceptance of authority, active listening.
