Position Description LPNs play an essential role in our facilities, providing vital care to residents. As a Licensed Practical Nurse, you will direct nursing assistants and perform day‑to‑day nursing duties to ensure residents receive comprehensive care.
Duties and Responsibilities
Direct the day‑to‑day functions of nursing assistants in accordance with current rules, regulations, and guidelines for long‑term care facilities.
Meet with nursing assistants to give a Nursing Report on assigned residents and review 24‑hour summary reports in the electronic medical record.
Perform administrative duties, including completing medical forms, evaluations, observations, events, and appropriate documentation for all assigned residents.
Ensure all medications and treatments have been signed off prior to the end of the shift and update the Nursing Supervisor on resident condition changes.
Receive telephone orders from physicians, transcribe orders, record them in the electronic health record, and prepare and administer medications as ordered.
Inform nursing staff of new admissions, expected arrival time, room assignment, and ensure rooms are ready for new admissions.
Make rounds with providers as necessary and conduct periodic checks to ensure resident care is completed by CNA/GNA and evaluate resident status.
Ensure nurse’s notes reflect that the care plan is being followed and review care plans for appropriate resident goals, problems, approaches, and revisions as needed.
Requirements
Current, unencumbered, active LPN license in your state or a compact state.
Current CPR certification.
Previous long‑term care experience is desirable.
Location: Newark, DE
Salary: $56,000.00–$86,000.00
