The Speech Language Pathologist (SLP) provides comprehensive skilled care as ordered by the attending physician within the specified geographical territory. The SLP educates patients and their families, ensures patient safety, and completes accurate documentation in compliance with all laws and regulations. The SLP operates within the speech-language pathologist scope of practice as outlined by state requirements. Qualifications

A current Speech-Language Pathologist (SLP) license in the state of employment. Minimum of one year of experience as an SLP is preferred. Home care experience is desirable. Effective communication and ability to establish positive impressions with patients, families, physicians and co-workers. Knowledge of speech therapy best practices and ability to make appropriate judgments. Ability to identify situations and determine appropriate solutions. Detail-oriented, observant, and timely with prioritization. Valid driver’s license and auto insurance with your name listed as a driver. Major Areas Of Responsibility

Provide skilled speech therapy as ordered by the attending physician. Evaluate patient functioning using diagnostic and prognostic procedures. Develop and adjust individualized speech therapy plans of care. Observe, record, and report patient reactions, changes, and progress to the attending physician and appropriate agency staff. Inform healthcare staff of phases of speech therapy relevant to patient care. Use activities and home exercise programs to improve swallowing, breathing, cognition, speech, and communication. Guide patient use of therapeutic and self-care activities to improve functioning. Promote continuity of care during admissions, transfers, and discharges. Counsel and instruct patients and families on disease or injury management across all phases of therapy. Involve patient and family in care plan development; instruct on procedures and equipment. Help reduce readmissions by teaching preventative measures and decision-making. Complete all clinical documentation per agency protocol and Medicare/Federal guidelines. Document all aspects of treatment, assessments, and patient education; perform chart audits as requested. Communicate changes to scheduler; participate in on-call requirements and case conferences. Attend in-service trainings, mandatory meetings, and mandated state programs; participate in orientation of new employees. Maintain professional appearance and adhere to agency policies and procedures. Health Qualifications

Must meet all physical position requirements. Evidence of a current negative TB test or x-ray, or TB testing at orientation; annual TB testing thereafter unless documented otherwise. Employee must be free of conditions that would limit job performance. Exposure to bloodborne pathogens is a possibility. Physical Demands: Travel required.
