What You’ll Do
Deliver
direct patient care
per State Nurse Practice Act
Conduct and follow up on
comprehensive assessments
and care plans
Accurately document patient services in a
timely
and detailed manner
Provide
education and support
to patients and families/caregivers
Coordinate hospice services
and communicate patient needs to the interdisciplinary team
Manage
pain and symptom control , evaluate treatment effectiveness
Qualifications
Graduate of an
accredited practical/vocational nursing program
3+ years of nursing experience
(hospice, community health, or med/surg preferred)
Current LPN/LVN license
in the state of practice
CPR certification
(active)
Strong
verbal, written, and organizational skills
Valid
driver’s license
and
auto insurance
as per state requirements
Understanding of
hospice philosophy
and the needs of the terminally ill
