Home Care Physical Therapist - Above and Beyond

and deliver evidence‑based physical therapy services in the comfort of patients’ homes.
Your Role As a Physical Therapist, you’ll provide personalized rehabilitation plans, assess progress, and collaborate with interdisciplinary teams to help clients regain independence and improve quality of life.
Key Responsibilities
Conduct comprehensive assessments and develop individualized treatment plans.
Deliver therapeutic interventions, including manual therapy, modalities, and exercise programs.
Educate clients, families, and caregivers on therapy techniques and home programs.
Supervise Physical Therapist Assistants (PTAs) and Home Health Aides (as permitted).
Collaborate with interdisciplinary teams and participate in care conferences.
Monitor progress, adjust treatment plans, and communicate changes to physicians.
Maintain accurate, timely documentation in compliance with regulations.

Qualifications
Degree from an APTA‑accredited Physical Therapy program.
Active state license to practice as a Physical Therapist.
1+ year of experience or completion of a 6–8 week preceptorship.
Home health experience and knowledge of Medicare Conditions of Participation preferred.
Valid driver’s license and reliable transportation.
Strong organizational and time‑management skills.
Physical & Mental Requirements
Ability to lift up to 35 lbs and assist with patient mobility.
Frequent standing, walking, bending, and positioning.
Adaptability to changing schedules and environments.
Travel to client homes and Trinity Health locations required.
