Enhanced Care Management (ECM) Lead Care Manager - San Joaquin County
Base pay range: $27.00/yr - $30.00/yr Location: Hybrid remote in San Joaquin, CA 95207 - On the road Job Type: Full-time Hours: 40 per week 8-Hour Shift, Monday to Friday, 8:30am PST - 5:00pm PST
Overview Pacific Health Group’s Enhanced Care Management (ECM) program focuses on addressing social determinants of health and providing community-based services. As a Lead Care Manager, you will guide members through care planning, arrange services, and build authentic relationships to support thriving in the community.
Responsibilities
Frequent in-person visits with members in homes, shelters, or community centers to assess needs, build trust, and address concerns
End-to-end care coordination: schedule doctor appointments, organize follow-up care, connect to social services, arrange transportation, housing, and other resources
Advocate for members, ensuring timely treatments and fair access to services; assist with insurance authorizations and appeals when needed
Maintain clear, detailed documentation of assessments, care plans, progress notes, and outcomes
Provide ongoing support via phone, video, and in-person visits; monitor progress and address new barriers promptly
Identify, coordinate, and optimize local resources (housing, job training, childcare, etc.) to support overall wellbeing
Stay current on Medi-Cal, CalAIM, and other regulations; ensure compliance with quality standards
Participate in professional development and trainings to improve cultural competence, motivational interviewing, and crisis intervention skills
Collaborate with the team to adapt care strategies based on member feedback and program needs
Qualifications (Requirements)
Residency: Must reside in San Joaquin County
Experience: 3-5 years in case management, social services, or healthcare
Expertise: Familiarity with Medi-Cal, CalAIM, and Enhanced Care Management
Healthcare Insight: Understanding of healthcare systems and local community resources
Interpersonal Skills: Strong communication, empathy, and cultural competence
Organizational Ability: Time management and attention to detail
Technical Proficiency: Experience with case management software and related tools
Successful completion of a pre-screen assessment
Driver's license, reliable vehicle for daily business use, and current liability insurance; may require MVR background check
