SSM Health Day Institute is seeking a Case Manager to work in our specialized outpatient rehabilitation program and have the ability to float within the region to assist other Day Institute facilities. We serve individuals recovering from a traumatic injury or illness who no longer require 24‑hour nursing or acute rehabilitative care. We take a comprehensive, multi‑disciplinary approach to build skills and maximize independence. Responsibilities
Monitors and assesses patient plan of care from pre‑admission to post‑discharge under general direction.
Assists patients/families in goal setting to maximize patient’s independence in both home and community.
Interacts with payers to obtain necessary authorizations for initiating and continuing care throughout admission to the program.
Qualifications
Professional healthcare degree in Nursing, Social Work, Licensed PT, OT, or SLP.
Preferred: experience in physical medicine and rehabilitation.
Salary: $80,000.00–$85,000.00 (St. Louis, MO)
