What You'll Do

As the Qualified Intellectual Disabilities Professional (QIDP), you will be responsible for the execution of each client’s Individual Program Plan (IPP), ensuring that all services are coordinated according to their unique service plan. You will also oversee the consistency and quality of services provided to each client.

Your Responsibilities Will Include

Managing casework for each individual enrolled in the ICF/DD program, ensuring continuity of care. Overseeing the implementation of individualized service plans through direct observation, documentation review, and collateral communication, in compliance with program policies and state regulations. Completing all required program documentation and reports promptly and accurately. Facilitate coordination and monitor services provided by external organizations (e.g., schools, day programs) through consistent progress reviews and participation in IEP/ISP meetings. Ensuring effective service delivery by contracted consultants, including Occupational Therapy, Physical Therapy, Speech Pathology, and Psychological Services. Maintaining consistent, transparent communication with Residential Manager, family members, the CSE chairperson, and clinical providers to support individuals across residential and educational settings. Ensuring that all staff members are adequately trained to meet the unique needs of each individual. Fostering collaboration across interagency departments while adhering to the established chain of command. Assisting in the referral process to alternative programs and working closely with families and social worker to support transition planning. Providing direct care assistance as needed to support program continuity and client well-being.

You’re a great fit for this role if you have:

Bachelor’s degree in a human services or related field Minimum of one (1) year direct experience in the care and training of persons with developmental disabilities Valid driver’s license (required)

Compensation

$24-$28 Per Hour
