Overview
Under the direction of the Oakland DRC Casework Supervisor, the DRC Case Manager is responsible for daily provision of direct services and case management activities to DRC parolees. The DRC Case Manager handles participant casework including Individual Treatment Plans (ITP), group and individual counseling, information and referral activities, and documentation of services. Using Motivational Interviewing and Cognitive Behavioral Therapy techniques, the DRC Case Manager ensures caseload participants have tools and skills to remain drug- and arrest-free through pro-social behaviors. The DRC Case Manager monitors progress through all phases of participation. Base pay range
$26.00/hr - $30.00/hr Responsibilities
Conduct screening and risk/needs assessment and face-to-face group and individual counseling for DRC participants; supervise DRC participants daily. Under supervision of the Casework Supervisor, develop and implement evidence-based rehabilitative interventions based on Motivational Interviewing and cognitive behavioral therapy techniques. Work with participants to develop and implement ITPs for participants through all program phases. Develop individual case files including assessment information, service plans, progress notes, participation tracking forms, discharge plans, releases of information, disclosure forms, and all required CDCR/DAPO documents. Provide service activities (18:1 ratio) as scheduled based on curriculum and daily activity schedule. Attend Multi-Disciplinary Team case management conferences and meetings as directed. Assist with monitoring vocational services activities. Facilitate groups and individual counseling related to required DRC program service requirements. Collaborate with other DRC program staff regarding participants on caseload. Maintain regular contact with Agents of Record regarding participant progress, discharge, and aftercare planning. Monitor and record participants’ Community Services requirements and activities. Monitor follow-through for referrals (Domestic Violence, housing, public benefits, health care, etc.) where applicable. Provide case management for aftercare services; identify other needed community-based support services and help participants build a community support network. Document participant progress and treatment activities through weekly progress notes and maintain accurate case files. Document participant issues of non-compliance using CDCR Title 15 and other guidelines and procedures. Conduct motivational interviewing sessions to address community reintegration issues aligned with ITP goals. Provide classroom and group management and supervision of participants. Negotiate modification of ITPs with the participant and Multi-Disciplinary Team. Assist Program Director and Casework Supervisor in training new staff. Monitor participant progress and provide reports to Casework Supervisor; evaluate progress and inform AOR of determinations. Participate in in-service training and cross-training; engage in Agency Work Force Development training. Maintain secure storage of all DRC participant records as instructed by Casework Supervisor. Seek formal weekly supervision with Casework Supervisor. Assist Job Developer in tracking participant job search activities. Track participant progress and self-study activities in obtaining GED where applicable. Submit participant requests to Casework Supervisor (transit vouchers, Caltrans Work Crew, etc.). Facilitate aftercare groups as directed. Ensure health, welfare, and safety of program participants. Follow CDCR/DAPO safety and security guidelines, policies, and procedures. Participate in peer chart reviews and provide services to a diverse participant population in a culturally sensitive manner. Assist with data collection activities and participate in Center Point trainings and continuing education. Observe required agency dress code and all personnel policies. Other duties as assigned. Minimum requirements
High School Diploma or equivalent and a minimum of six months working with rehabilitative services to criminal justice populations. Basic knowledge of Cognitive Behavioral Therapy, Motivational Interviewing Techniques, Psycho-Educational interventions, and 12-Step programs. Must meet CDCR clearance requirements. If an ex-offender, must be successfully discharged from parole or probation for at least three years prior to hire. Must have current TB clearance. Knowledge of applicable ethical and professional standards of conduct. Good written and verbal communication skills. Intermediate computer skills. Possesses a valid California Driver’s License. Demonstrated experience in group and individual counseling, case management, documentation, treatment plan development, and development of evidence-based rehabilitative interventions. Demonstrated experience in developing targeted treatment outcomes and strategies. Experience working with a culturally diverse population in a multidisciplinary staff. Ability to promote culturally appropriate and gender-responsive services. Physical Demands
