Under the supervision of the Director of Nursing Services, the Medicare/MDS Coordinator is responsible for notifying and coordinating the Interdisciplinary Team (IDT) for MDS assessment completion in accordance with State and Federal regulations. Coordinate the Medicare/MDS resident assessment process. Ensure timely completion of the MDS Assessment by the Interdisciplinary Team. Develop, implement, and evaluate the plan of care. Coordinate and perform treatments, medications, or nursing interventions as indicated by the resident's care plan or physician's orders. Provide nursing care in accordance with infection control standards. Follow safety policies during nursing care. Initiate emergency measures as needed according to center policy and nursing standards. Requirements: Current LVN License in California. At least 2 years of experience as an MDS Coordinator in a Skilled Nursing Facility. Background check with no history of abuse.
