FPATH Outreach and Engagement Specialist

Kitsap Mental Health Services (KMHS) is hiring an FPATH Outreach and Engagement Specialist to provide community-based outreach, engagement, and support to individuals impacted by the criminal legal system. As part of Washington State’s response to the Trueblood settlement, the Forensic Projects for Assistance in Transition from Homelessness (FPATH) program plays a critical role in supporting individuals with serious behavioral health needs who are experiencing or at risk of homelessness. This position helps participants transition from incarceration or instability into recovery-oriented services, housing, and treatment, improving outcomes and reducing recidivism. The Outreach and Engagement Specialist will partner with courts, jails, law enforcement, housing providers, and internal KMHS teams to build trusted relationships, reduce barriers, and guide individuals toward lasting stability. Responsibilities

Manage and respond to incoming referrals; track and assign cases efficiently. Carry a caseload of clients needing intensive outreach and engagement as the primary intervention. Provide telephonic and in-person outreach to assist clients in addressing barriers to service connection. Collaborate with FPATH staff, clinical teams, and external agencies to improve workflows and care coordination. Facilitate pre-engagement activities such as problem-solving, resource education, appointment reminders, and follow-ups. Conduct outreach with Forensic Navigators in jail or community settings to build rapport with potential enrollees. Represent the FPATH program in courtrooms, stakeholder meetings, community forums, and system-level planning efforts. Serve as liaison to law enforcement, jails, attorneys, and housing authorities; maintain positive relationships to support client access. Deliver presentations and training to community partners on the FPATH model and eligibility. Support participants in transitions from incarceration or unstable housing into permanent housing, behavioral health treatment, or other supports. Collaborate with clients to build individualized service and housing stabilization plans focused on long-term success. Promote service delivery that is strength-based, trauma-informed, and recovery-oriented. Provide crisis intervention as needed, including during outreach and re-engagement activities. Support clients in attending orientation, initial appointments, and addressing barriers such as transportation or interpreter needs. Provide outreach to community locations including courts, hospitals, emergency departments, and homeless shelters. Document all client interactions and progress in the KMHS electronic health record in a timely and accurate manner. Minimum Qualifications

Education:

Bachelor’s Degree in Psychology, social services, or behavioral health field. Experience:

Agency Affiliated Counselor Registration. Note:

This position requires driving clients in a personal vehicle on behalf of the agency; therefore, the incumbent must meet agency driving requirements, including meeting the minimum age requirement of 21 years old, successfully completing a motor vehicle history check, possessing and maintaining a current, valid driver's license in the state of Washington, and having reliable, insured transportation. Preferred Qualifications

Experience: 2 years of job-related experience. Experience in co-occurring disorders treatment, individual and group therapy, vocational services, or substance abuse treatment. Experience interacting with several external agencies including law enforcement, legal teams including attorneys and legal advocates. Performance Requirements

Knowledge:

Maintain a working knowledge of current trends in community mental health, including health care reform and automation of job tasks using computer technology; knowledge of mental health rehabilitation standards. Skills:

Establishing and maintaining effective working relationships with other employees, clients, organizations, and the public; clear and concise communication; computer skills to operate a PC, fax machine, printer, and copier. Abilities:

Demonstrated ability to meet or exceed productivity standards; work cooperatively as a team member; build positive working relationships with staff of all agency departments. Work Environment

Frequent mobility and/or sitting required for extended periods. The role may involve driving a car in outreach contexts.
