Program Description Felton Institute’s Early Psychosis award winning program (re)MIND® (formerly PREP) was originally developed in partnership with the University of California San Francisco (UCSF). Felton’s (re)MIND® is an early psychosis coordinated specialty care model for early detection and treatment of schizophrenia spectrum disorders. (re)MIND® core evidence-based practices include Cognitive Behavioral Therapy for Psychosis (CBTp), Strength-Based Care Management, Family and Peer Support, Medication Management and Individual Placement and Support (IPS) model of supported education and employment. The (re)MIND® model has three additional components, Felton BEAM, BEAM UP, and (re)MIND Alumni, respectively for individuals diagnosed with bipolar or mood disorders with psychosis, for individuals at clinical high risk of developing psychosis, and for early psychosis aftercare services. Funded through partnerships with county behavioral health departments, Felton's Early Psychosis Division operates outpatient coordinated specialty care for early psychosis programs in San Francisco, Alameda, San Mateo, and Marin counties, as well as the innovative (re)MIND® Central program, which expands access to early psychosis coordinated specialty care.

Job Description The Care Manager II works as part of a multidisciplinary team and is responsible for coordinating participant care within the team and with community partners. This includes implementing treatment plans collaboratively with participants and team members and providing therapeutic interventions that target psychosis, mood, and additional issues that may impact recovery (such as trauma, substance use, interpersonal problems, and environmental stressors, etc.). The Care Manager II is expected to provide services to participants in both the office and in the community (homes, schools, libraries, etc.). Local travel expected. The Care Manager II reports directly to the (re)MIND® Clinical Program Manager.

Specific Responsibilities

Provide case management, lead groups focused on skill-building, crisis intervention, and harm-reduction support for participants struggling with early stages of psychosis or early stages of bipolar disorder.

Provide direct support to assist participants with challenges in the areas of independent living skills, wellness, housing, personal goals, socialization, and connection to their community.

Assist participants to identify employment/education goals, strengths, barriers, and steps towards goals.

Help participants build employment and education skills including focus, motivation, follow through, resumes and applications, communication, engagement, interviewing, etc…

Support participants in advocating for their needs and potentially interface with participant’s employer/school staff to facilitate participant’s success. Provide resources and information to employer/school staff as appropriate and with support of clinicians.

Connect to long term supports for employment and education resources and supports.

Assist participants in increasing independence by navigating public transportation to school or work.

Work collaboratively with the treatment team, community providers and social service agencies to help maintain stability, promote recovery and/or mental and health care supports.

Utilize Motivational Interviewing, Cognitive Behavioral Therapy skills, and other evidence-based and evidence-informed practices suitable for diverse TAY participants.

Support participants with accessing appropriate community-based resources, including accompanying participants to appointments and following up with community-based organizations to advocate and record participant outcomes.

Actively participate in outreach events, conferences, open houses, etc..., and provide information about (re)MIND® program to other mental health providers and the general public as needed.

Meet the required number of direct service contact hours per week and document all participant contacts in compliance with mandated quality assurance standards.

Participate actively in supervision, coaching, data collection and reporting, staff meetings, intake and discharge activities, case conferences and all agency meetings and trainings as assigned by the program leadership.

Develop and maintain appropriate relationships and boundaries with participants.

Carry a caseload of assigned program participants.

Adheres to laws regarding confidentiality and reporting requirements and maintains necessary compliance documentation.

Availability to work some evenings and/or weekends with advance notice.

Other duties as required.

Minimum Requirements and Qualifications

Bachelor's degree in Social Work, Counseling Psychology, or a closely related field and two years of experience in a behavioral health, social services or a community services organization (Care Manager II). Bachelor's degree may be substituted with AA degree in related field + 3 years of experience, or 5+ years of experience working with TAY populations. Candidates with higher qualifications such as Master’s degree in Social Work, Counseling Psychology, graduate trainee-level or BBS associate-level applicants (ASW, AMFT, APCC) may be considered and qualify for adjusted compensation.

Experience with public and private mental health services, community resources and TAY client population preferred.

Ability to provide services in a variety of settings and excellent professional skills for time management and use of technology (EHRs, MS Office 365, cloud-based storage, video conferencing platforms).

Knowledge of county behavioral health departments and community standards.

Knowledge of State and Federal requirements for clinical documentation and reporting, including MediCal documentation standards.

Valid California Drivers’ License with an excellent driving record.

Bilingual/Bicultural and fluent in Spanish or threshold Asian languages preferred.

FLSA Status: Exempt

Salary Range: $80,000 - $100,000 Annually (DOE)
