POSITION SUMMARY
This evening shift
covers the 4 pm to 12 am shift. During this shift, the LVN is available to travel to visit patients at their residence and is compensated for the visits made. Under the direction of a Registered Nurse, the hospice LVN provides skilled nursing and palliative care to patients at their residence. The LVN works cooperatively with other Interdisciplinary Team members and provides intermittent visits or shifts of skilled nursing care, including pain management/symptom control, nursing observations, and patient education. The LVN participates in care plan development and implementation. Must be comfortable with electronic medical records and utilizing computers.
QUALIFICATIONS Graduate of an accredited school of vocational nursing. Current LVN license within the State of California. Certification in Hospice and Palliative Nursing encouraged. At least one year of LVN experience. Valid driver's license, automobile insurance, reliable and well-maintained transportation. Spanish speaking is a plus but not required.
