JOB SUMMARY:

Works closely with the Special Projects Coordinator and other Case Manager III in the Homeless Services division to help homeless families and individuals obtain transitional housing or rapid rehousing. Performs screenings, intakes, and other direct service deliveries for clients. Provides intensive case management to all households to identify barriers to housing. Works with clients to develop a self-sufficiency plan. They determine barriers to self-sufficiency, set mutually agreed-upon objectives for clients, and evaluate clients’ progress to modify the plan and services. Offers supportive services, such as job training, food, transportation, and referrals to other services, to assist participants. Learn about SHARE’s partner agencies and their services to help clients as needed. Enters information into the Homeless Management Information System (HMIS). Attends Upstate Continuum of Care and HUD meetings as needed. Develops key relationships with landlords and property managers. Maintain accurate records of all customer assistance and compile customer records for SHARE and HUD reports. Provides monthly SHARE board reports on program activities and outcomes. Performs inspections of all housing units for transitional and rapid rehousing programs. Collaborates with the Accounting Department to request checks for rent and utility assistance, as well as other necessary purchases. Issues purchase orders according to the agency’s Fiscal Policies and Procedures. Assists the Special Projects Coordinator in compiling all required reports for SHARE and HUD. Performs other duties as assigned.

QUALIFICATIONS:

Must have a Bachelor's degree in Social Work or a related field. Preferred qualifications include licensure as a social worker or an equivalent combination of formal training and related experience. Must have at least two years of work experience providing direct services to low-income, very low-income, and/or homeless populations. Knowledge of intake and assessment techniques is preferred. Must have good verbal and written communication skills. The ability to work independently without close supervision is required. The ability to plan and execute a daily work schedule is required. Excellent organizational and recordkeeping skills are required. Legible handwriting and computer skills are required. The ability to work in high-stress situations is required. Good coping skills are required. Knowledge of service areas and community resources is preferred. A valid South Carolina driver's license and an acceptable driving record are required. Dependable transportation is required.
