Job Summary:
The Clinical Case Manager will facilitate the continuum of care for an assigned number of cases by managing, coordinating, facilitating, and monitoring resources to achieve optimal clinical outcomes, utilization of services, and financial goals. The Clinical Case Manager oversees the discharge process for all patients within the service area and is an integral team member in efforts to establish a safe and appropriate discharge plan/plan of care. Minimum Qualifications:
NYS licensed RN (or eligible), Bachelor's degree in Nursing or a healthcare-related field, plus a minimum of three (3) years acute care RN or acute care, transitional care, or chronic care RN/case management experience OR Associate's degree in Nursing and five (5) years acute care RN or acute care, transitional care, or chronic care RN/case management experience required. If Associate in Nursing, must be enrolled in an accredited BSN program with an anticipated graduation date within the next 2 years. Will be appointed temporary until receipt of BSN. CPR-BLS certification is required by the time of appointment. Preferred Qualifications:
Master's degree in Nursing or healthcare-related field and prior acute care case management experience preferred. General medical/surgical experience preferred; PRI certification a plus. Work Days:
Monday-Friday, with some weekend and holiday coverage.
