Medical Social Worker – Inpatient Care Coordination Presbyterian Healthcare Services

Compensation Base pay: $26.21/hr – $40.03/hr. Sign-on and relocation bonuses available for qualified candidates.

Job Summary Presbyterian is seeking a Medical Social Worker to work with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources, and qualify for community assistance from a variety of special funds and agencies. The role includes providing crisis intervention to patients and families, coordinating and facilitating the development of a discharge plan of care for all patient populations, and receiving referrals and consults for patients from at-risk populations from interdisciplinary team members.

Responsibilities

Participate in and arrange discharge planning activities to ensure a timely discharge and provide appropriate linkage with post-acute providers.

Work with families exhibiting complex dynamics that directly impact patient care and discharge.

Collaborate with case managers and multidisciplinary team members regarding discharge planning and arrange the appropriate level of care needed.

Educate patients/families and physicians regarding post-acute options and address issues of choice.

Actively participate and provide pertinent information in complex rounds, discharge-planning huddles, and unit daily whiteboard rounds.

Serve as lead in assisting patients/families to obtain available financial and other resources such as Medicaid, SSDI, county indigent funds, UNM Cares, etc.

Provide intervention in child abuse/neglect, domestic violence, guardianship, foster care, adoption, mental health placement, advance directives, adult/elderly abuse, child protection, and sexual assault.

Assist as indicated with end-of-life matters.

Meet CMS Condition of Participation requirements and applicable state and federal regulations.

Obtain legal guardianship and competency determinations, completing and disseminating required legal documentation.

Provide and update community services resources, advice, and referral information.

Promote professional growth by meeting mandatory and continuing education requirements and supporting department goals.

Document assessments, interventions, and discharge planning results in the medical record per departmental policies and procedures.

Qualifications Social Worker III

Master’s degree in Social Work required.

Current license in New Mexico (LISW, LCSW, or LMSW).

Ten years of hospital social work or discharge planning experience required.

LMSW preferred.

National Case Management certification preferred.

Social Worker II

Master’s degree in Social Work required.

Current license in New Mexico (LISW, LCSW, or LMSW).

Five years of hospital social work or discharge planning experience required.One to two years of hospital social work, protective services, or practicum experience desired.

National Case Management certification preferred.

Social Worker I

Master’s degree in Social Work required.Current license in New Mexico (LISW, LCSW, or LMSW).

One to two years of hospital social work or discharge planning experience required.

One to two years of hospital social work, protective services, or practicum experience desired.

National Case Management certification preferred.
