We are currently seeking a Nurse Practitioner for our Palliative Care team. This role works in collaboration with our hospice staff and under the direction of the Director of Palliative Care to provide high-quality, patient-centered palliative care. The NP will conduct visits with new and established patients in a variety of settings—including private homes, facilities, and hospitals—while helping patients and families navigate their care with compassion and expertise.
Key Responsibilities
Conduct comprehensive patient visits and develop individualized plans of care.
Provide medical expertise in symptom management, prognosis, and palliative treatment.
Perform Medicare-required “face-to-face” encounters as needed.
Collaborate with interdisciplinary team members, attending physicians, and community providers.
Educate patients and families on disease process, care choices, and symptom management.
Serve as a resource and advocate for palliative care within the community.
Participate in team meetings, quality initiatives, and on-call rotation as needed.
Qualifications
Nurse Practitioner licensure in the Commonwealth of Virginia (required).
At least 2 years of experience as an NP in acute care, home health, hospice, palliative, or geriatric nursing (required).
Board certification in hospice and palliative medicine (preferred).
Fellowship in hospice and/or palliative medicine (preferred).
Valid driver’s license and reliable insured automobile (required).
Strong clinical assessment skills, independence in decision‑making, and ability to work collaboratively.
Schedule: Monday through Friday, 8:30 am to 5 pm, but please note that hours outside of these may require a call on some evenings and weekends.
