$41,600.00 to $48,958.00 per year. This is an exempt (salaried) position. Pay rates are based on education, skill, experience level and internal equity. Community Support Services (CSS) transforms lives and community by supporting veterans, ending homelessness, treating substance use, and preventing and intervening in community and domestic violence. As a department in Mecklenburg County’s Health and Human Services Agency, CSS maximizes community partnerships and collaborative opportunities to improve access to services and advocate for the needs of special populations. Position Specific Information

Shelter Plus Care is a federally grant funded permanent supportive housing program that links housing with supportive services to move individuals, and adults with families, who are homeless, have a disability and a low-income, to permanent housing. Support for participants combines a rental assistance subsidy funded by the U.S. Department of Housing and Urban Development's Continuum of Care Program, and supportive services administered and provided by Mecklenburg County Community Support Services and other community partners. This position assists households enrolled in Shelter Plus Care in housing navigation which includes determining fair market rate, rent calculations, housing search, and securing lease agreements. It also includes landlord recruitment and retainment, assisting households in mediating landlord/tenant issues. Position Details:

Full time onsite Monday through Friday, 8 am to 5 pm. Schedule flexibility needed for special projects and/or service and department needs. Office located at 3205 Freedom Drive, Charlotte, NC 28208. Position Summary Responsible for providing professional-level case management or case coordination services for various populations. This position, under general supervision, will participate in treatment planning and the creation and implementation of a person-centered plan. Essential Functions

Review cases and provide supporting documentation regarding clients receiving financial assistance Interview and screen potential clients; review mental health and/or substance abuse history; conduct case consultations with applicable individuals; coordinate discharge planning Evaluate consumer’s immediate circumstances, including environmental situations, availability of services/support and develop service plans Prepare a variety of reports and documents related to client services, progress, and/or other related consumer issues and ensure all documentation adheres to State policies and procedures Foster inter-agency collaborations and research additional services or programs available to clients Coordinate new referrals and eligibility evaluations Develop Individualized Family Service Plans with families to address concerns, needs and priorities Arrange, link and integrate services with families Maintain compliance with procedural safeguards including client confidentiality and parental rights Enter and maintain progress notes and clinical services in computerized chart and conduct chart reviews Monitor client(s) progress on a regular basis via home/community visits, or phone contacts Assist clients in resolving concerns related to community resources, issues involving treatment or rehabilitation, and various living problems in home, work, or social environments; provide support as consumer copes with negative consequences of specific mental health issues May be required to staff shelters in the event of a disaster Minimum Qualifications Experience: Minimum of two years in an applied setting such as mental health, substance use, or other health conditions, or in community related service Education: Bachelor’s degree in Social Work, Psychology, Counseling, Sociology or any related human services field Combination of relevant education and relevant experience accepted Licenses and Certifications: Requires a valid North Carolina or South Carolina Driver's License Requires County Driving Privileges Annual Motor Vehicle Record (MVR) check required Preferred Qualifications Master’s degree in a human services related field preferred Current or previous experience with homeless populations and Housing First model Familiarity with trauma informed care and Harm Reduction model Some accounting background helpful and experience with grants Knowledge, Skills and Abilities Knowledge
